Primary Care Nurse

Position Overview
The role of the Primary Care Nurse is to provide safe and exceptional nursing services within a primary care setting. Adhering to the NMC Code of Conduct, the nurse will implement health interventions tailored to the patient population.
Key Responsibilities
Clinical Care Delivery:
- Administer medications via injection, ensuring compliance with legal requirements.
- Conduct cervical cytology sampling.
- Identify health promotion opportunities and provide concise lifestyle advice.
- Assess patients with wounds, managing care independently or referring as necessary.
- Remove closure materials such as sutures and staples.
- Collect samples for cervical screening in accordance with established standards.
- Administer vaccines to both adults and children following the UK immunisation schedule.
- Offer pre-travel health consultations, including vaccine administration and risk assessment.
- Provide prescribed therapies, recognizing potential side effects and contraindications.
- Establish and follow referral pathways for patients with specialized needs.
Management of Long-Term Conditions:
- Oversee nurse-led clinics for long-term conditions.
- Perform clinical assessments and annual reviews for patients with chronic conditions such as diabetes, asthma, and COPD, adhering to NICE guidelines.
- Recognize signs indicating potential undiagnosed long-term conditions and mental health issues, utilizing appropriate referral pathways.
Confidentiality:
Maintain strict confidentiality regarding patient information, ensuring that sensitive data is only shared with authorized personnel in accordance with practice policies.

Promote health and safety in the workplace by following infection control procedures, maintaining equipment, and identifying risks associated with work activities.
