Key Responsibilities:- Ensure the quality of care and safety of our clients and carers.
- Lead and support Nurse Case Managers within the region to ensure all care is safe and quality-driven.
- Hold a small caseload while supporting the growth of the business with assessing and onboarding new clients and delivering clinical training.
- Build strong relationships with commissioners, community multidisciplinary teams, and clients and families.
- Lead and manage Nurse Case Managers, ensuring they understand their responsibilities and accountabilities.
- Experience in a similar role or as a Nurse Case Manager.
