Position Overview

We are seeking a dedicated Band 3 Rehabilitation Support Specialist to become a vital member of our Therapy Team. This role is integral to our efforts in enhancing patient care within the Medicine division. Our focus is on creating a supportive environment for patients undergoing rehabilitation, ensuring a seamless transition from hospital to home.

Key Responsibilities

The successful candidate will collaborate closely with the multidisciplinary team (MDT) on the ward and in community settings. Your primary duties will include:

  • Assisting patients in engaging with therapeutic activities as they prepare for discharge.
  • Supporting patients with daily living tasks, including personal care and meal assistance, fostering a culture of prompt discharge.
  • Working alongside Occupational Therapists to facilitate smooth transitions into community care.

This position is ideal for an enthusiastic and adaptable individual, preferably with experience in rehabilitation or caregiving, although this is not a strict requirement.

Essential Qualifications

Candidates should possess:

  • Relevant healthcare experience, demonstrating effective MDT collaboration.
  • A passion for supporting patients and a commitment to enhancing their rehabilitation journey.
  • QCF or NVQ level 2 or equivalent qualifications in Maths and English.
  • An understanding of rehabilitation processes and the roles of various healthcare professionals.
  • The physical capability to assist patients with mobility and handle therapeutic equipment in accordance with safety guidelines.
  • Strong communication skills, both written and verbal.
