An exciting opportunity has emerged within our organization for a Rehabilitation Case Coordinator with expertise in sports therapy or rehabilitation. This role involves leveraging clinical experience in a telephonic and office-based setting to effectively support individuals in their recovery journey. The ideal candidate will demonstrate exceptional communication skills to guide and assist patients, while maintaining meticulous attention to detail in a dynamic work environment with multiple priorities.

Key Responsibilities

Clinical Service Delivery

  • Conduct thorough and consistent telephone assessments that lead to optimal clinical outcomes.
  • Evaluate, plan, implement, and monitor appropriate interventions and clinical pathways to facilitate patient recovery and case resolution.
  • Employ the bio-psychosocial model in patient interactions and clinical decision-making.
  • Utilize every patient interaction as an opportunity to educate and provide clinical support.

Patient Care

  • Collaborate with colleagues on patient cases, fostering a supportive team environment.
  • Contribute to enhancing service delivery.
  • Uphold the highest standards of patient care, ensuring accurate clinical guidance and clear communication of terms and conditions.
  • Adopt a proactive, patient-centered approach to navigate case management challenges.
Essential Qualifications

Required:

  • Relevant clinical degree (sports therapy, rehabilitation, or physiotherapy).
  • Minimum of one year of clinical or hands-on postgraduate experience, or prior case management expertise.
  • Current registration with a relevant professional body (e.g., BASRaT, SST, HCPC) or ability to obtain membership promptly.
  • Strong written and verbal communication skills, along with excellent attention to detail and time management abilities.

Preferred:

  • Experience in delivering high-quality customer service and/or proven case management experience.
  • Relevant background in health, legal, or insurance sectors.
  • Membership in CMS UK.
