About the Role:We are seeking a highly skilled and experienced physiotherapist to join our case management and rehabilitation team. As a Clinical Case Manager, you will work with vulnerable children, young people, and adults who have experienced life-changing injuries or illnesses.
Your key responsibilities will include:
- Managing a caseload of clients through their rehabilitation journey
- Leading on assessments in the home and developing rehabilitation plans
- Coordinating care and support services, including therapy and rehabilitation, financial support, and educational needs
- Working closely with clients, families, and other professionals to achieve successful rehabilitation outcomes
To be successful in this role, you will need:
- A valid clinical registration with an appropriate clinical body (e.g., HCPC, NMC, SWE)
- Previous experience in case management or working in the community setting
- Strong relationship-building skills and a willingness to travel extensively across the UK, Scotland, and Wales
