Physiotherapist - Case Manager

About the Role:
We are seeking a highly skilled and experienced Part-Time Children's Physiotherapist to join our team as a Clinical Case Manager. As a Clinical Case Manager, you will play a vital role in supporting children, young people, and adults with life-changing injuries and illnesses.
Key Responsibilities:
- Manage a caseload of clients through their rehabilitation, leading on assessment and goal setting.
- Coordinate relevant care and support, including accessing statutory services, therapy, and rehabilitation.
- Implement plans for successful rehabilitation with clear, manageable goals.
- Work closely with clients, their families, and other professionals to ensure seamless care and support.
Requirements:
- A valid clinical registration with an appropriate clinical body (e.g., HCPC, NMC, SWE).
- Previous experience establishing or implementing rehabilitation packages.
- Strong relationship building skills and ability to work with diverse clients and families.
- Willingness to travel extensively across the UK, Scotland, and Wales.
