About the Role

We are seeking a highly motivated and organized Care Coordinator to join our team at Cestria Health Centre. As a Care Coordinator, you will play a vital role in supporting patients with complex needs and coordinating their care across health and social services.

Key Responsibilities
  • Proactively identify and support individuals with complex needs, including the frail, elderly, and those with long-term conditions.
  • Collaborate with clinical and non-clinical teams to ensure patients receive the support they need.
  • Empower patients to manage their conditions and access necessary services, enhancing their quality of life.
  • Work closely with social prescribing link workers to provide a comprehensive approach to personalized care.
  • Engage with diverse populations from various cultural and social backgrounds.
Requirements
  • Demonstrate an appreciation of the health needs of the patient population within the PCN.
  • Experience of working in primary care or a health-related field.
  • Excellent communication and interpersonal skills.
  • Organized, patient, and empathetic approach to patient care.
