Patient Care Facilitator

Position Overview
The Patient Care Facilitator will be an integral member of a dynamic multi-disciplinary team within a healthcare setting, dedicated to assisting patients across various practices in their network. This position offers an opportunity to influence the development of services and the role itself. You will engage with patients to help them comprehend their health conditions and identify what is important to them, aligning available local support to promote their well-being and independence. Responsibilities may include collaborating with healthcare professionals managing local care facilities, organizing multi-disciplinary team meetings for both clinical and non-clinical staff, or directly working with specific patient groups, such as the elderly or those with chronic health issues.

Key Responsibilities
As a Patient Care Facilitator, your duties will include:
1. Identifying and supporting individuals, particularly the elderly and those with long-term health conditions.
2. Coordinating and navigating care across health and social services, focusing on vulnerable patients and enhancing health in care settings.
3. Collaborating with both clinical and non-clinical teams to ensure individuals receive the necessary support.
4. Empowering patients to manage their health and access essential services, thereby improving their quality of life.
5. Working alongside social prescribing link workers to provide a holistic approach to personalized care.
6. Engaging with diverse communities from various cultural and social backgrounds.
The Patient Care Facilitator will be a motivated, forward-thinking professional with:

- Experience in health, social care, or support roles involving direct interaction with individuals, families, or caregivers.
About Our Organization
We operate across multiple sites and have established partnerships with various healthcare professionals, including general practitioners and nurse practitioners, who also conduct home visits. Our in-house pharmacy team significantly reduces administrative burdens, allowing us to focus on patient care.

Qualifications and Experience
Essential
- A clear understanding of the health needs of the patient population.
Desirable
- Previous experience in primary care or a GP practice.
- Familiarity with the local healthcare landscape.
- Knowledge of safeguarding principles and local health and social care resources.
- Experience working with vulnerable or elderly patients.
Required Qualifications
