PCN Care Coordinator

We are seeking a highly organized and detail-oriented PCN Care Coordinator to join our team at PartnersHealth. As a PCN Care Coordinator, you will play a vital role in supporting the delivery of primary care services in the Rushcliffe area.
Key Responsibilities:- Run reports from clinical systems such as SystmOne to analyze appointment data and understand capacity and demand.
- Book patient appointments and ensure patient clinical records are accurate and up-to-date.
- Provide administrative support to GP practice teams, including generating reports and managing staff rotas.
- Support patients to book appointments and manage their needs by answering queries and providing clear and accessible information.
- Work closely with GP practice teams to coordinate clinics and multi-disciplinary meetings.
- Maintain PCN and GP Practice websites and waiting room screens.
- Use clinical systems for record-keeping, audit, and evaluation.
- Ability to work accurately and calmly under pressure in a busy environment.
- Excellent communication skills, including the ability to work effectively with patients, GP practice teams, and other stakeholders.
- Ability to work independently and as part of a team, with minimal supervision.
- High level of organization and administrative skills, with the ability to prioritize tasks effectively and manage workload.
- Ability to maintain confidentiality and adhere to organizational policies and procedures.
