About the Role

We are seeking a highly skilled and experienced Band 6 Community Practitioner to join our Pulmonary Rehabilitation Team at Dorset County Hospital NHS Foundation Trust. As a key member of our team, you will play a vital role in delivering high-quality patient care and contributing to the development of our service.

Key Responsibilities
  • Undertake all aspects of clinical duties, including assessing patients' suitability for the Pulmonary Rehabilitation program and managing patient exercise and education sessions.
  • Work at an advanced level, prioritizing your own work area and making decisions without direct supervision.
  • Collaborate with other members of the respiratory MDT, including DAIRS team and Respiratory Champions in Primary Care.
  • Develop and maintain close links with other healthcare professionals to ensure seamless patient care.
Requirements
  • Diploma or Degree in Physiotherapy, Occupational Therapy, or Nursing.
  • Registered as a Physiotherapist/Occupational Therapist with HCPC or as a nurse with the RCN.
  • Experience in rotational physiotherapy/occupational therapy or nursing, including experience in the specialist area.
  • Knowledge of evidenced-based practice pertaining to Pulmonary Rehabilitation and national guidelines and standards.
