Job Summary

We are seeking a highly skilled and experienced Registered Mental Health Nurse/Community Practitioner to join our team at Nottinghamshire Healthcare NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to patients living in the community, working closely with clients, carers, and other healthcare professionals to deliver personalized care plans and specialist interventions.

Key Responsibilities
  • Manage a caseload of patients living in the community, acting as a care coordinator and providing comprehensive need/risk assessments, treatment plans, and interventions.
  • Establish and maintain robust communication networks with clients, carers, team members, and other healthcare professionals to ensure seamless care delivery.
  • Use interpersonal skills to diffuse potentially emotive situations while undertaking assessments and pathway planning in acute settings.
  • Collaborate with other agencies and primary care to ensure timely and safe discharge from secondary care.
Requirements
  • Professional Qualification (Mental Health) to degree level or equivalent.
  • Maintains current registration with the NMC.
  • Further specialist knowledge/post-registration experience in specific service areas or other formal training.

  • Awareness of national and local issues impacting on nursing care.
  • Knowledge of Management theories.
  • High level of emotional intelligence.
Contractual Requirements
  • Ability to work to meet the needs of the service.
  • A full driving license and vehicle for business use.
