About the Role
We are seeking a skilled Health and Wellbeing Coach to support patients with long-term conditions in managing their health and wellbeing. As a key member of our team, you will work closely with GPs and multidisciplinary teams to develop and implement patient care plans.
Main Responsibilities
- Provide personalized care and wellbeing coaching to patients, empowering them to understand and manage their conditions independently.
- Work collaboratively with healthcare professionals to develop and implement patient care plans.
- Participate in multidisciplinary team meetings to discuss patient progress and share best practices.
- Engage with community resources and initiatives to promote healthy lifestyles and support patients in achieving their goals.
- Manage a caseload independently, ensuring patient outcomes are met and quality improvement projects are supported.

Requirements
- Excellent communication and interpersonal skills, with the ability to build rapport and establish good one-to-one relationships quickly.
- Ability to deal with challenging behavior and create professional boundaries.
- IT skills and experience working with databases.
- Coaching qualification and experience working with patients with long-term conditions.
Qualifications
- English and Maths GCSE or equivalent.
- Coaching qualification and relevant experience.
