About the Role
This is an exciting opportunity to work in a dynamic and supportive environment, where no two days are the same. As a Lead Practitioner, you will be responsible for:
- Providing individualized support to individuals with a wide range of needs, while working in a team environment.
- Supporting individuals to engage with their hobbies and interests, and empowering them to develop skills and independence.
- Attending community groups and accompanying individuals to activities they enjoy.
- Assisting with personal care, ensuring individuals' comfort and wellbeing.

Requirements
To be successful in this role, you will have:
- A genuine commitment to supporting individuals to live their best lives.
- Previous experience in care, with a focus on supporting individuals with complex needs.
- Excellent communication and interpersonal skills.
- The ability to work flexibly, including evenings and weekends.
- A level of physical fitness, as the role may require manual handling and lifting.
