Job Title: Senior Mental Health Practitioner

We are seeking a highly skilled and experienced Senior Mental Health Practitioner to join our Adults and Health team. As a key member of our team, you will be responsible for providing high-quality care and support to vulnerable adults in the community.

Key Responsibilities:

  • Undertake holistic assessments and develop personalized support plans for service users.
  • Provide mental health act assessments and work with service users to avoid hospital admission.
  • Work collaboratively with other professionals to ensure seamless care and support.
  • Develop and maintain strong relationships with service users, families, and carers.
  • Contribute to the development of the team and service through participation in quality improvement initiatives and professional development opportunities.

Requirements:

  • Recognized social work qualification and registration with Social Work England.
  • Experience of working with vulnerable adults and mental health issues.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of relevant legislation and policies, including the Care Act and Mental Health Act.
