Job Title: Older Adult CMHT

Are you a dedicated and compassionate Community Psychiatric Nurse looking to make a meaningful difference in the lives of older adults? We have an exciting opportunity for a CPN to join our Older Adult Community Mental Health Team (CMHT) in Greater Manchester.

Key Responsibilities:

  • Provide high-quality patient care to older adults with mental health needs
  • Work collaboratively with a multidisciplinary team to develop and implement individualized care plans
  • Conduct assessments, develop treatment plans, and provide ongoing support to patients and their families
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Participate in team meetings and contribute to the development of team policies and procedures

Requirements:

  • Current and valid registration with the NMC
  • Proven experience in a CPN recovery role
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Commitment to providing high-quality patient care
