Position Overview
We are seeking a motivated and skilled Band 6 Physiotherapist to become a vital member of our Rapid Response team, which is part of the broader intermediate care service.
This role primarily involves:
- Providing Urgent Community Response for non-injury fallers and urgent equipment needs.
- Supporting patients with complex conditions in their homes to prevent hospital admissions.
- Collaborating with A&E departments to facilitate rapid patient turnaround.
- Assisting with discharge assessments for patients requiring short-term rehabilitation.
Key Responsibilities
The primary goal of the Rapid Response Service is to empower patients to remain in their homes, recover effectively, and engage with their daily lives while minimizing the need for acute care admissions.
The successful candidate will utilize advanced assessment and clinical management skills to support patients identified for Rapid Response intervention, aiming to prevent further health deterioration or hospital stays. Close collaboration with local neighborhood teams will be essential.
Your responsibilities will include:
- Conducting specialized therapy assessments to avert hospital admissions.
- Engaging in Urgent Community Response activities, including assisting patients in regaining mobility.
- Facilitating quicker discharges from local A&E departments through effective treatment planning and risk assessment.
- Promoting independent living and rehabilitation through a person-centered approach.

Qualifications and Experience
Essential qualifications include:
- A diploma or degree in Physiotherapy.
- HCPC registration.
- Membership in the Chartered Society of Physiotherapy (CSP).
- Evidence of continued professional development.
- A valid driving license with appropriate business insurance.
- Experience in community and clinical rehabilitation, particularly with elderly patients.
Desirable qualifications include:
- A postgraduate course in elderly care or related fields.
- Membership in relevant CSP interest groups.
