About the Role

The Clinical Coordinator will be responsible for the effective management of the Rapid Response Coordinating Centre, ensuring the smooth operation of the service and the delivery of high-quality patient care.

Key Responsibilities

  • Manage the day-to-day operations of the centre, including the triaging of referrals and the allocation of resources.
  • Lead a team of healthcare professionals, providing clinical supervision and guidance as needed.
  • Develop and implement service development and transformation plans, including audits and quality improvement initiatives.
  • Ensure that all staff in the team act in accordance with Trust HR and other relevant policies, along with the appropriate professional code of conduct.
  • Deputise for the line manager in their absence and may be required to undertake clinical visits.

About the Team

The Rapid Response team is a dynamic and multidisciplinary team that works closely with other healthcare services to provide urgent care to patients in their own homes. The team is made up of Admin, Healthcare Support Workers, Student Nurses/Occupational Therapists/Physio Therapists, Trainee Nursing Associates, Assistant Practitioners, Trainee Assistant Practitioners, Rehab Assistants, Paramedic, Registered Nurses, Occupational Therapists and Physiotherapists.

Person Specification

Essential

  • Relevant degree or equivalent qualification and appropriately registered with the NMC or HPC as per post requirements.
  • Demonstrable leadership experience, which enables the post holder to function at a Band 6 Level.
