This is an exciting opportunity to join NHS Professionals Limited as a Community Psychiatric Nurse, working in a community setting and providing crucial mental health support and interventions directly within the community.
Key Responsibilities- Provide robust assessments of patients who have been referred due to presenting with mental health difficulties.
- Identify appropriate care pathways and ensure the least restrictive option to meet the needs of the patient and ensure their wellbeing.
- Signpost to appropriate care providers, including primary and tertiary care.
- Raise concerns to senior staff for support and management.
- Complete documentation and planning care for patients.
- Present cases to Multi-Disciplinary Team for discussion around care pathway.
- Liaise with colleagues nursing, occupational therapy, medics, psychology and admin.
