Position Overview

We are pleased to announce an opportunity for an Advanced Nursing Practitioner at the forefront of care within the Cleft Lip and Palate service. This role is integral to our multidisciplinary team, providing specialized support and guidance to patients and their families.

Key Responsibilities

The Advanced Nursing Practitioner will:

  • Serve as the primary contact for a designated patient caseload, ensuring a seamless experience through the care continuum.
  • Offer expert advice, education, and emotional support to patients, caregivers, and healthcare colleagues.
  • Deliver clinical services in accordance with established protocols across various care environments.
  • Actively participate in the efficient operation of the Cleft service, collaborating with a diverse team of healthcare professionals.
  • Coordinate with hospital and community services to facilitate integrated care pathways for patients and their families.
  • Engage in the continuous improvement of the Cleft service, contributing to research and audit initiatives.
  • Provide specialized nursing care and psychological support, advocating for the needs of patients throughout their treatment journey.
  • Assess and address the health and psychosocial requirements of patients, fostering shared decision-making.
  • Empower patients and families through effective communication, enabling informed choices regarding their health.
  • Maintain comprehensive patient records in compliance with regulatory standards and contribute to clinical data collection.
  • Collaborate with other healthcare professionals to enhance care delivery and patient outcomes.
  • Participate in the development of care standards and guidelines to ensure high-quality service delivery.
  • Support educational initiatives for patients, families, and staff, promoting health literacy and self-care.
