Emergency Care Practitioner
Position Overview
An exceptional opportunity has arisen at Hedena Health Ltd for an Emergency Care Practitioner to become a vital member of our Acute Hub team within a rapidly expanding and innovative healthcare environment.
We are in search of dedicated and passionate Emergency Care Practitioners to join our well-established, welcoming, and supportive team. Our commitment is to provide the highest standards of care for our patients, collaborating within a diverse multi-disciplinary team that includes General Practitioners, GP trainees, Practice Nurses, Paramedics, Pharmacists, Social Prescribers, Community Health Workers, and Mental Health Professionals. Our innovative approach to patient care, along with our focus on teamwork and personal/career development, is equally important.
If you are new to Primary Care or currently engaged in this field, we are eager to support your professional growth through the HEE Advanced Practice Roadmap. For those already qualified as Advanced Practitioners, we will continue to provide support through annual study leave, continuing professional development sessions, and regular team training. We value the right attitude, personality, and your ability to demonstrate commitment, intelligence, and forward-thinking.
Subject to funding and eligibility, we are enthusiastic about supporting staff in becoming non-medical prescribers. If this aligns with your aspirations, we would be delighted to connect with you.
Main Responsibilities
The primary focus of this role will be to work within the Practice's Acute team. This team is dedicated to treating patients of all ages with acute health issues, ideally on the same day, and often serves as the first point of contact for patients seeking care.
In addition to the Acute Hub, there is an opportunity to engage with our developing integrated neighborhood team, which supports patients in the community and provides both acute and chronic care at home to help prevent hospital admissions.
You will be responsible for delivering and maintaining a high standard of care to patients while assisting the Practice with various tasks as required. This includes assessment, diagnosis, treatment, and referral of patients based on their individual needs.
In the course of your duties, you will:
- Maintain accurate clinical records for all patient assessments, ensuring timely documentation.
- Provide evidence-based and patient-centered care, ensuring that the dignity, confidentiality, and wishes of patients are always respected, while working autonomously within your scope of practice and seeking support when necessary.
- Ensure the safety of yourself, colleagues, and patients by remaining vigilant about infection prevention and control, adhering to practice policies.
