About the Role

We are seeking a highly motivated and compassionate Community Outreach Support Worker to join our team at Longhurst Group. As a Community Outreach Support Worker, you will play a vital role in supporting individuals with Learning Disabilities and/or Mental Health needs in the community.

Key Responsibilities
  • Maximise the independence of individuals requiring support, enabling them to make real choices in their lives.
  • Support customers to engage and participate in their local community, as well as maintain settled accommodation.
  • Provide support to ensure strong economic wellbeing, including helping customers access benefits entitlements, pay rent and household bills, and manage debt.
  • Assist, support, and prompt customers to live independently.
  • Maintain accurate records of support provided, risk assessments, and support plans.
  • Work collaboratively with partner agencies, such as Birmingham City Council and localised NHS services.
Requirements
  • Experience working in a similar role supporting individuals with enduring mental health needs.
  • Knowledge and understanding of Mental Health and Learning Disabilities, as well as the issues surrounding them.
  • Ability to challenge negative beliefs and provide positive behaviour support.
  • Strong communication and listening skills, with the ability to provide advice and support to customers.
  • Full driver's licence and access to a vehicle to travel to customers across Birmingham.
