About the Role

The Royal Devon and Exeter NHS Foundation Trust is seeking a highly skilled and experienced Advanced Clinical Practitioner to join our Stroke Rehabilitation Unit as a key member of our multidisciplinary team. As a Clinical Practitioner, you will play a vital role in the development and leadership of the rehabilitation beds on the stroke rehabilitation unit, including admission, discharge, and patient care.

Key Responsibilities
  • Develop and lead the rehabilitation beds on the stroke rehabilitation unit, ensuring high-quality patient care and outcomes.
  • Work closely with our Consultant Physiotherapist to facilitate the development of a coordinated service for patients with stroke, and for those needing stroke-rehabilitation within the area.
  • Maintain and develop relationships with external partners, including specialist rehabilitation units, to optimize transitions of care.
  • Be accountable for the delivery of stroke rehabilitation, ensuring that patients receive the best possible care and support.
Requirements

We are looking for a suitably qualified applicant with a passion for the holistic, multidisciplinary care and rehabilitation of patients who have experienced a stroke. Prescribing/deprescribing is an essential part of the role, so the post is unlikely to suit individuals with an Occupational Therapy background.
