We are seeking a highly skilled and experienced Mental Health Clinical Practitioner to join our team at Rotherham Doncaster and South Humber NHSFT. As a key member of our Primary Care Network, you will play a vital role in providing high-quality mental health interventions to patients in our community.
Key Responsibilities- Provide specialist advice and support to Primary Care Networks in triaging mental health concerns.
- Support GPs in making appropriate referrals and bridging the gap between primary and secondary care services.
- Act as a first point of contact for GPs to gain advice and support regarding patients' mental health.
- Liaise with GP teams and Primary Care Networks to address complex situations.
- Participate in primary care practice level meetings, including those involving serious concerns, safeguarding, and risk management.
- Provide a comprehensive Primary Mental Health Nursing Service across the whole range of patients, including bio-psycho-social assessment and treatment delivery.
- Work autonomously and hold Independent Non-medical Prescribing accreditation, or be prepared to work towards it immediately.

Requirements- NMC Registered Nurse or registered allied professional (OT/Social work).
- Degree-level registration qualification plus equivalent post-registration qualifications or experience to postgraduate level.
- Independent Non-medical Prescribing accreditation (or willingness to work towards it).
- Registered Mental Health nurse.
- Teaching and Assessing Qualification.
- Psycho-Social Interventions (PSI) Qualification.
- Non-medical prescriber.
- Experience of working in a broad range of care settings and services.
- Experience of working with individuals with a breadth of physical and mental health conditions.
- Effective communication, leadership, improvement, and change management skills.
- Ability to work independently and as part of a multi-professional team.
- Confidence to discuss patient management and express clinical opinion with other team members.
- Willingness to challenge and address ineffective practice and systems.
- Strong clinical role model and leader, inspiring others to deliver safe and effective care.
