We are seeking an experienced Advanced Clinical Practitioner to join our team at Wentworth 1 Primary Care Network. As a key member of our primary care network, you will be responsible for delivering high-quality patient care and working collaboratively with our multidisciplinary team to improve patient outcomes.
Main Responsibilities- Work autonomously and be accountable for your own professional actions, utilizing advanced clinical skills and expert knowledge to deliver holistic care to patients.
- Assess undifferentiated patients with a range of acute, non-urgent, and chronic medical conditions, utilizing your expertise to formulate differential diagnoses and develop management plans.
- Undertake pre-booked emergency clinics, face-to-face consultations, telephone, or video consultations, and work collaboratively with our team to ensure seamless patient care.
- Participate in screening patients for disease risk factors and early signs of disease, and refer patients directly to other services or agencies as appropriate.
- Work with our multidisciplinary team to promote integrated and seamless pathways of care, and contribute to the development of our primary care network.

Person Specification- Registered Adult Nurse, Paramedic, or Pharmacist with the relevant governing body.
- Independent non-medical prescriber with a minimum of 2 modules at Level 7, including assessment of clinical examination skills.
- Experience of working in primary care, with a broad and sound knowledge of complex and common conditions in adults and children.
- Consolidated independent prescribing experience and use of examination skills in practice.
