Overview of the Position:

Assist individuals from their initial engagement with the service throughout their rehabilitation journey. Collaborate with the Shared Care General Practitioner to help individuals attain lasting recovery. Liaise with local General Practitioner with Special Interests (GPSI) regarding substance misuse treatment within Primary Care settings. Partner with community Pharmacists to ensure effective communication regarding supervised consumption arrangements, engagement, and medication collection. Advocate for sexual health awareness, offering guidance on protective measures and risk reduction, and facilitate screening in conjunction with the Practice Nurse. Conduct screening, evaluation, recovery planning, and referrals as necessary. Aim to diminish drug and alcohol-related harm to individuals and the broader community. Encourage involvement from carers, service users, and the community. Provide support for access to collaborative services. Work alongside individuals to foster social reintegration, enabling them to lead fulfilling and purposeful lives by promoting recovery, resilience, peer support, and self-determination.

Candidate Profile:

Possess a comprehensive understanding of substance misuse challenges and relevant experience in a similar environment. Demonstrate sound knowledge of clinical prescribing matters. Have experience in community-based support methodologies. Exhibit strong communication skills, both verbal and written, along with proficient IT capabilities. Be proactive and capable of working independently, with excellent time management and prioritization abilities. Maintain a solid understanding of mental health interventions, services, and best practices. Be a collaborative team member with strong interpersonal skills, capable of partnering with a diverse range of professionals, agencies, and stakeholders. Show the ability to manage change effectively while prioritizing the needs of service users. Recognize the significance of information governance processes and commit to adhering to all necessary safeguards. Actively seek learning opportunities to enhance and expand professional knowledge and skills, while also contributing to the learning and development of others.
