About the Role

We are seeking a highly motivated and experienced Health and Wellbeing Support Worker to join our team at North Kerrier East Primary Care Network (PCN). As a key member of our team, you will play a vital role in supporting individuals to take proactive steps to improve their physical and mental health conditions.

Key Responsibilities
  • Manage and prioritize your own caseload to support individuals in improving their health and wellbeing.
  • Use coaching skills and motivational interviewing to empower individuals to take control of their health and wellbeing.
  • Develop and contribute to multi-disciplinary team meetings to effectively manage cases and provide personalized care plans.
  • Support individuals to access self-management education courses, peer support, and other interventions to improve their health and wellbeing.
  • Work collaboratively with clinicians, mental health practitioners, and care coordinators to provide holistic support to individuals.
Requirements
  • Good level of education and relevant qualifications in health coaching, motivational interviewing, or a related field.
  • Experience of working in health, social care, education, or other support services with direct contact with individuals, families, and professionals.
  • Experience of working with vulnerable adults and children, and ability to work positively with individuals facing complex social and emotional challenges.
  • Ability to use a range of tools and techniques to enable and support individuals, such as agenda setting, goal setting, and problem solving.
  • Ability to prioritize your own workload and meet deadlines.
