About the Role

We are seeking an experienced Cardiothoracic Nurse to join our Clinical Nurse Specialist team within the Transplant Unit at Harefield Hospital. As a key member of our Advanced Nursing Team, you will provide individualized care to patients and their families/carers throughout the transplant process.

Key Responsibilities
  • Lead the provision of care in accordance with Trust Policies/Protocols, reflecting current trends and developments in nursing and the NHS.
  • Provide care, education, and support for designated patient caseloads, acting as a specialist resource when liaising with multidisciplinary teams.
  • Assess, plan, implement, and evaluate programs of care for transplant assessment/waiting list patients, utilizing evidence-based practice to improve the quality of care and meet patient needs.
  • Facilitate transplant assessment/waiting list out-patient clinics and telephone review clinics.
  • Network with other Transplant Coordinators throughout the UK, collaborating, sharing, and benchmarking practices and initiatives.
People Management
  • Act as a role model, demonstrating high standards of care and providing clinical leadership in the specialist area.
  • Manage own annual leave, in conjunction with colleagues, and in line with Trust and local Policy and Procedure.
Service Development
  • Develop transplant-related information for patients, families, and other healthcare professionals.
  • Be responsible for data entry and accurate maintenance of the transplant waiting list and relevant transplant databases.
  • Plan, lead, and delegate local audit and research as required, ensuring findings are disseminated appropriately and recommendations implemented.
Research and Audit
  • Participate in the collection of audit and research data.
  • Encourage other staff to recognize the need for nursing research.
  • Lead on nursing research, including publishing papers and speaking at conferences/meetings.
  • Participate in introducing and applying research findings to enhance patient care.
