Position Overview

We are seeking a Change Management Lead to join our organization and take a crucial role in enhancing health and wellness services across Primary Care Networks (PCNs) and the Mid Notts area. This position involves leading transformation initiatives, collaborating with Clinical Directors, PCN members, and various community stakeholders to foster integration and preventive care strategies.

Key Responsibilities

  • Collaborate with relevant colleagues and partner organizations to identify and implement enhancements in the quality and efficiency of primary care services.
  • Engage with PCN members and local partners to facilitate integrated neighborhood initiatives.
  • Support transformation efforts aimed at improving local healthcare services, aligning with strategic objectives outlined in the Primary Care Strategy and other relevant plans.
  • Implement Population Health Management strategies to address health disparities and promote proactive, personalized healthcare across communities.
  • Provide guidance to clinical leaders to ensure the successful execution of strategic goals and initiatives.
  • Utilize change management methodologies to define the scope and benefits of transformation projects, ensuring alignment with organizational objectives.
  • Contribute to the strategic planning of projects, identifying interdependencies, potential impacts, resource needs, and contingency strategies.
  • Foster strong working relationships with colleagues across the healthcare landscape.
  • Oversee the management of staff as necessary.
