About the RoleThis is a dynamic and challenging role that requires a strong clinical background in Gerontology, with a focus on delivering innovative and patient-centered care. You will work closely with our multidisciplinary team to develop and implement new pathways of care, improve patient outcomes, and enhance the overall quality of life for our patients.
Key Responsibilities- Contribute to the development and implementation of new pathways of care for older adults with complex medical needs
- Work closely with our multidisciplinary team to deliver high-quality care and improve patient outcomes
- Participate in the General Unselected Medical Take, weekend Frailty and Gerontology support, and out-of-hours Gerontology telephone on-call rota
- Contribute to the admission avoidance rapid access 'HOT' clinics provided by the department's Consultant Connect service
- Play a significant role in service redesign and quality improvement initiatives
To be successful in this role, you will need to have a strong clinical background in Gerontology, with a focus on delivering high-quality care to older adults. You will also need to have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced and dynamic environment.
