Lead Mental Health and Substance Misuse Outreach Team

About the Role
Solent NHS Trust is seeking a dedicated and experienced Clinical Manager to lead our newly established Co-occurring Conditions Outreach Team. This innovative team will focus on providing timely and effective support to individuals in Portsmouth experiencing both mental health conditions and substance misuse.
As the Clinical Manager, you will play a pivotal role in shaping the direction of this team, fostering collaboration with internal and external partners, and ensuring seamless access to services for those in need. Your leadership will be instrumental in developing pathways and protocols that enhance joint working between mental health and substance misuse services.
Key Responsibilities:
- Provide strategic direction and management oversight to the Co-occurring Conditions Outreach Team, ensuring alignment with organizational goals and best practices.
- Cultivate strong relationships with a diverse network of stakeholders, including statutory agencies, voluntary organizations, and community partners.
- Champion a collaborative and integrated approach to care, facilitating seamless transitions between services and promoting holistic support for individuals.
- Supervise and mentor team members, providing guidance, support, and opportunities for professional development.
- Monitor service performance, identify areas for improvement, and implement strategies to enhance effectiveness and efficiency.
About You:
We are seeking a highly motivated and passionate individual with a proven track record in mental health and substance misuse services. You will possess exceptional leadership skills, a commitment to person-centered care, and a strong understanding of the complexities faced by individuals with co-occurring conditions.
Essential Qualifications:
- Registered Mental Health Nurse (NMC), Social Worker, or Occupational Therapist.
- Management or leadership qualification and/or relevant training and experience.
Experience Required:
- Demonstrated experience in effectively managing complex and high-risk cases within a mental health setting.
- Significant post-qualification practitioner experience in both mental health and substance misuse services.
- Proven ability to work collaboratively within a multidisciplinary team environment.
- Experience as a supervisor, including a clear understanding of managing patient flow and risk assessment tools.
