About the Role

We are seeking a highly skilled and dedicated Community Staff Nurse to join our team at Buckinghamshire Healthcare NHS Trust. As a key member of our community healthcare team, you will be responsible for providing high-quality nursing care to patients in their homes, promoting independence and improving health outcomes.

Key Responsibilities
  • Contribute to the development and implementation of care plans, working closely with patients, families, and other healthcare professionals to ensure seamless care delivery.
  • Assess and manage patient needs, providing evidence-based interventions to improve health outcomes and quality of life.
  • Collaborate with multidisciplinary teams to ensure effective communication and coordination of care.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices.
  • Develop and maintain strong relationships with patients, families, and community partners, promoting trust and confidence in our services.
Requirements
  • Registered health professional with a Diploma level qualification.
  • Experience in community nursing, with a strong understanding of patient needs and care delivery.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Ability to work under pressure, prioritizing tasks and managing multiple responsibilities.
