Southern Health is collaborating with other NHS Trusts to create a new, integrated organization – Hampshire and Isle of Wight Healthcare NHS Foundation Trust. About the Role: Mental Health Practitioner

We are seeking a dedicated and enthusiastic Registered Social Worker, Nurse, or Occupational Therapist to join our Adult CMHT team. If you are passionate about making a difference in the lives of individuals with mental health challenges and eager to advance your career, this opportunity is for you.

Your Responsibilities:

  • Actively participate as a member of our multidisciplinary team, providing comprehensive assessment and care planning for service users.
  • Act as a care coordinator for a designated caseload of individuals with severe and enduring mental health conditions.
  • Conduct ongoing assessments, implement treatment plans, and evaluate the effectiveness of interventions.
  • Deliver therapeutic interventions individually or within group settings.
  • Collaborate with other agencies to ensure holistic recovery-focused care.
  • Produce reports, complete audits, and conduct clinics as required.
