About the Role

We are seeking a highly skilled and compassionate Mental Health Practitioner to join our Early Intervention in Psychosis Team at Southern Health NHS Foundation Trust. As a Care Coordinator, you will play a vital role in supporting individuals with psychosis and their families, providing expert care and guidance to ensure the best possible outcomes.

Main Responsibilities

  • Work closely with individuals with psychosis and their families to develop personalized care plans
  • Conduct comprehensive assessments and risk assessments to identify areas of need
  • Develop and implement treatment plans, including medication management and therapy
  • Collaborate with multidisciplinary teams to ensure seamless care coordination
  • Provide education and support to individuals and their families on psychosis and recovery

Requirements

  • Registered Mental Health Nurse, Mental Health Practitioner, OT, or Social Worker
  • Full driving license and ability to meet travel requirements
  • Knowledge of psychosis and experience working with individuals with mental health conditions
