We are seeking a skilled Mental Health Practitioner to join our team in Rural West Primary Care Network (PCN) and Southern Health NHS Foundation Trust. As a Mental Health Practitioner, you will play a vital role in supporting and delivering transformation work around No Wrong Door.
Key Responsibilities:
- Work with Primary Care Network to support and help deliver the transformation work around No Wrong Door.
- Support the planning and coordination of service users' care in primary care, including those on the SMI list, liaising with teams and agencies as required.
- Discuss primary care referrals that require referral to secondary mental health care prior to making a referral.
- Deliver therapeutic interventions to service users whose immediate needs may require help in a primary care setting.
