Role Overview

Your Responsibilities

The Community Care Supervisor plays a vital role in ensuring the delivery of top-notch care support services. This includes a variety of responsibilities such as assessing and reviewing individual care plans, providing practical supervision, and offering guidance to care staff. You will collaborate with care coordinators to develop effective care worker schedules that cater to the unique needs of clients while considering the specific skills of the care team.

Regular reviews of care services will be part of your duties, including conducting needs and risk assessments. You will also participate in multi-disciplinary evaluations of service users' requirements and mentor new care staff in the field to ensure their proficiency. While the majority of your time will be spent in the community, you will also engage in essential office-based administrative tasks.

Qualifications

Essential Requirements

Experience in conducting initial risk and needs assessments for individual service users is necessary, whether in their homes or other settings such as hospitals or care facilities. You will work collaboratively with service users, their families, caregivers, healthcare professionals, and others to ensure the seamless delivery of comprehensive care packages.
