About the Role

We are seeking a dedicated and caring Support Worker to join our team at Community Integrated Care. As a Support Worker, you will play a vital role in empowering people with support needs to live the lives they want to lead.

Key Responsibilities
  • Provide person-centred support to individuals with a range of disabilities and care needs, including autism, mental health, physical disabilities, diabetes, epilepsy, acquired brain injuries, and behaviours that may challenge.
  • Support individuals to achieve their everyday goals, including personal care, and help them to participate in activities and hobbies that promote their well-being.
  • Work collaboratively with colleagues to ensure that individuals receive high-quality, consistent care and support.
  • Develop and maintain positive relationships with individuals, their families, and other stakeholders to ensure that their needs are met.
  • Contribute to the development and implementation of individual care plans, ensuring that they are tailored to meet the unique needs of each person.
Requirements
  • Self-motivated with energy and drive to provide high-quality support.
  • Reliable, friendly, and caring approach to supporting individuals with complex needs.
  • Ability to work collaboratively as part of a team to achieve shared goals.
  • Confident and inspiring approach to supporting individuals to achieve their goals.
  • Ability to manage challenging behaviours in a safe and respectful manner.
  • Comfortable using basic IT systems to record and report on individual progress.
