About the Role

We are seeking a highly skilled and experienced Community Rehabilitation Physician to join our team at Oxford Health NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality medical care to patients in our community hospital settings.

Key Responsibilities
  • Proactive assessment of new admissions to the ward, including medical clerking, medication review, and prescribing, as well as escalation planning.
  • Performing Comprehensive Geriatric Assessments alongside the multidisciplinary team to optimize patient health and working with patients, their families, and the MDT to create advanced care plans.
  • Responding to medical concerns as they arise, including history taking, examination, investigation, diagnosis, treatment, and onward referral when appropriate.
  • Planned formal weekly review of all patients on a ward round and involvement in weekly MDT meetings.

Person Specification
  • Full GMC registration with a license to practice and up-to-date with appraisal and revalidation requirements.
  • Active NHS clinical experience in General Practice with demonstrable experience in working with urgent care and older people.
  • Ability to work in a high-functioning MDT with respect for the roles and experience of other team members.
