Job Description

Job Title: Acute Medicine Doctor

Job Summary: We are seeking a skilled and experienced General Practitioner to join our Community Hospital Rehabilitation and Frailty Team at Oxford Health NHS Foundation Trust.

Main Responsibilities:
  • Medical Assessment and Care Planning: Conduct proactive assessments of new admissions to the ward, including medical clerking, medication review, and prescribing, as well as creating advanced care plans with patients and their families.
  • Comprehensive Geriatric Assessment: Perform comprehensive geriatric assessments alongside the multidisciplinary team to optimize patient health and create effective care plans.
  • Medical Concerns and Escalation: Respond to medical concerns as they arise, including history taking, examination, investigation, diagnosis, treatment, and onward referral when necessary.
Team Collaboration and Communication:
  • MDT Meetings and Board Reviews: Attend multidisciplinary team meetings and board reviews to lead on patient recovery and timely discharge.
  • Collaborative Working: Work collaboratively with colleagues outside the hospital setting who work in our Intensive Community Care and First Contact Care Pathways.
Administrative and Governance Responsibilities:
  • Administrative Duties: Undertake clinically related administrative duties, such as reviewing investigation results and writing discharge letters.
  • Clinical Governance and Audit: Participate in the organization and delivery of clinical governance, audit, and quality improvement activities as required.
Requirements and Qualifications:
  • Essential Qualifications: Completion of GP training (either JCPTGP certificate or MRCGP).
  • Desirable Qualifications: Higher degree or equivalent in a relevant field of medicine, postgraduate teaching qualification, or relevant postgraduate diplomas.
Personality and Skills:
  • Collaborative Mindset: Ability to work in a high-functioning multidisciplinary team with respect for the roles and experience of other team members.
  • Leadership Skills: Ability to work within a collaborative multidisciplinary framework and share leadership responsibilities with senior colleagues of other professions.
