Dementia Care Manager

Hallmark Care Homes is seeking a highly skilled and compassionate Dementia Care Manager to lead our dementia communities in providing exceptional, relationship-centred care to our residents.
Key Responsibilities- Develop and implement a comprehensive dementia strategy that aligns with Hallmark's quality and compliance framework.
- Lead and manage a dedicated team of caregivers to deliver evidence-based care that promotes the well-being and dignity of our residents.
- Collaborate with the Head of Relationship Centred Care and Dementia Relationship Practitioner to ensure seamless delivery of care services.
- Build and maintain strong relationships with local community groups and organizations involved in dementia care, promoting Hallmark's commitment to excellence.
- Support family members and friends of residents with dementia, empowering them to become active partners in their loved one's care.
- Ensure the dementia community meets all CQC requirements, including assessment, care planning, quality assurance, health and safety, and recruitment and management of staff.
- Passion for dementia care and a commitment to delivering exceptional, relationship-centred care.
- Proven leadership and management skills, with experience in managing a team of caregivers.
- Strong communication and interpersonal skills, with the ability to build and maintain relationships with residents, families, and colleagues.
- Ability to work collaboratively as part of a multidisciplinary team, promoting a culture of excellence and continuous improvement.
- Knowledge of CQC requirements and Hallmark's quality and compliance framework.
