Key Responsibilities- Lead and manage the dementia community, developing and implementing Individual Care Plans, Risk Assessments, and Daily Assignment Sheets.
- Deliver dementia-related learning and support to team members and relatives, ensuring compliance with CQC regulations and Hallmark standards.
- Develop and implement strategies to improve dementia care, working closely with the management team to achieve our goals.
- Collaborate with other departments to ensure seamless delivery of care services, promoting a culture of excellence and professionalism.
- Conduct regular audits and assessments to ensure compliance with regulatory requirements, identifying areas for improvement and implementing changes as needed.
- NVQ 4 or 5 / QCF 5 in Health and Social Care or willingness to work towards it.
- At least 4 years experience in long-term care of the older person, including dementia care.
- Detailed knowledge of care plans and end-of-life care, with a strong understanding of adult abuse, safeguarding, and CQC regulations.
- Academic qualification in Management and Leadership or equivalent experience, with a proven track record of success in a similar role.
- A competitive salary ranging from up to £43,000 annually, depending on experience.
