Job Title: Advanced Clinical Research Practitioner

We are seeking an experienced Advanced Clinical Research Practitioner to join our team at Healthier West Wirral Primary Care Network. The successful candidate will be responsible for providing high-quality patient care and contributing to the development of our care home service.

About the Role

The Advanced Clinical Research Practitioner will work closely with our multidisciplinary team to deliver proactive care to patients in our care homes. This will involve conducting ward rounds, reviewing patient records, and developing personalized care plans. The successful candidate will also be responsible for maintaining accurate documentation and patient records, and for communicating effectively with colleagues, patients, and their relatives.

Key Responsibilities
  • Conduct ward rounds and review patient records to identify areas for improvement
  • Develop and implement personalized care plans for patients in our care homes
  • Maintain accurate documentation and patient records
  • Communicate effectively with colleagues, patients, and their relatives
  • Participate in multidisciplinary team meetings and contribute to the development of our care home service
Person Specification
  • Registered healthcare professional with an MSc in Advanced Clinical Practice or equivalent
  • Independent Non-Medical Prescriber registered with their relevant professional body
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Strong problem-solving and analytical skills
