Job Description

Job Summary:

The Advanced Clinical Practitioner will utilize specialist knowledge and skills to provide healthcare autonomously to patients in care homes within the Healthier West Wirral Primary Care Network. The ACP is accountable for independent, competent clinical decision-making, including assessment and diagnostic skills, for patients within our care homes.

Main Responsibilities:

  • Conduct proactive care rounds in care homes to deliver high-quality care, reviewing routine follow-ups and patients recently discharged from hospital.
  • Create and review personalized care and support plans for each resident, identifying residents of concern for preventative/proactive admission avoidance.
  • Work alongside the multidisciplinary team to conduct proactive care planning, including DNACPRs, Advance Care Plans, RESPECT, and Emergency Health Care Planning, to improve care and reduce avoidable admissions.
  • Manage a caseload of complex clients using advanced problem-solving skills and clinical reasoning.
  • Maintain accurate documentation and patient records, liaising with supervising GPs, community services, and secondary care as required.
  • Complete on-ward referrals as needed to work alongside and support the multidisciplinary team, attend educational sessions, and prescribe medications and treatments in accordance with clinical guidelines and protocols.

Key Requirements:

  • Registered healthcare professional with an MSc in Advanced Clinical Practice or equivalent at MSc level.
  • Independent Non-Medical Prescriber registered with their relevant professional body.
  • Advanced level of knowledge and competence in all aspects of acute, community, primary, and social care medicine.
