Mental Health Triage Nurse

We are seeking a highly skilled and experienced Mental Health Triage Nurse to join our team at Central and North West London NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing comprehensive assessments, triage, and referrals for individuals with mental health needs.
Key Responsibilities- Conduct thorough assessments of patients' mental health needs, including risk assessments and care planning.
- Develop and implement individualized care plans, working closely with patients, carers, and other healthcare professionals.
- Provide high-quality, evidence-based interventions, including psychosocial therapies and medication management.
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
- Participate in multidisciplinary team meetings to discuss patient care and develop treatment plans.
- Contribute to the development and implementation of policies and procedures related to mental health care.
- Current Professional Registration as a Registered Mental Health Nurse.
- Experience working with individuals with complex mental health needs, including those with acute/severe and enduring mental health problems.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and other healthcare professionals.
- Ability to assess, plan, implement, and evaluate comprehensive care packages for individuals with mental health needs.
- Knowledge of relevant legislation, policies, and procedures related to mental health care.
