About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team at Central and North West London NHS Foundation Trust. As a Primary Care Mental Health Triage Practitioner, you will play a vital role in providing high-quality care to patients with mental health needs.

Key Responsibilities
  • Assess patients presenting with mental health difficulties and develop comprehensive care plans.
  • Work closely with GPs, practice nurses, and other healthcare professionals to provide seamless care.
  • Screen referrals, identify patients who require specialist mental health services, and facilitate onward referrals.
  • Provide short-term interventions for patients with complex mental health needs.
  • Support the safe discharge of patients from secondary care and ensure effective continuing care in primary care settings.
  • Develop and maintain strong relationships with GPs, practice nurses, and other stakeholders to promote collaborative working.
  • Participate in regular supervision and contribute to the development of the team.
Requirements
  • Current professional qualification in mental health nursing or occupational therapy.
  • Registration with the relevant professional body.
  • Post-registration training and continuing professional development (CPD) in mental health.
  • Experience of working in multidisciplinary mental health teams with clients with acute and severe mental health problems.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
  • Ability to assess, plan, implement, and evaluate comprehensive care packages for patients with mental health needs.
