Job Description:
The Health Club is seeking an experienced Band 6 Community Mental Health Nurse to join our team on a locum basis for a 3-month contract. As a Community Mental Health Nurse, you will be working within the Older Persons Intensive Crisis Support Team, providing high-quality care and support to individuals in need.
Responsibilities:
- Provide mental health assessments and interventions to individuals in the community
- Develop and implement individualized care plans to meet the unique needs of each client
- Collaborate with multidisciplinary teams to ensure comprehensive care and support
- Conduct regular reviews and evaluations to assess the effectiveness of care plans
Requirements:
- Degree or equivalent in Mental Health Nursing
- Experience working as a Mental Health Nurse
- NMC Registration
