About the Role

We are seeking an experienced and skilled Senior Community Mental Health Nurse to join our team at South West London and St Georges Mental Health NHS Trust. As a key member of our community team, you will be responsible for providing high-quality care to patients with mental health conditions in their own homes and within various accommodation settings.

Key Responsibilities
  • Assessing and treating patients with mental health conditions, including those with complex needs.
  • Providing a range of interventions, including health promotion, social support, and medication optimization.
  • Coordinating care with other professionals, including psychiatrists, psychologists, and social workers.
  • Leading and managing the work of others, including junior staff and community team members.
  • Ensuring a flexible approach to care provision, with a focus on choice and social inclusion opportunities.
Requirements
  • Registered Adult Nurse and/or Registered Mental Health Nurse and/or Registered Learning Disabilities Nurse.
  • Mentorship qualification.
  • Significant relevant clinical experience, including experience of research-based/reflective practice.
  • Knowledge of clinical governance and audit, as well as relevant Professional and Clinical legislation.
