Clinical Support Nurse

Job Summary
The successful candidate will join a dynamic team of healthcare professionals at a large, busy GP Practice in Wales, serving over 25,000 patients across three sites. As a Practice Nurse, you will play a vital role in delivering high-quality patient care, working closely with GPs, HCAs, and other healthcare staff to provide comprehensive support in areas such as diabetes management, respiratory care, women's health, and family planning.
Main Responsibilities
The post-holder will be responsible for undertaking a range of duties and responsibilities, including:
- Providing chronic disease management, including diabetes, respiratory, and women's health
- Wound management and care
- Immunisation and vaccination programs
- Women's health and family planning services
- Adherence to infection control procedures
- Taking and recording observations, such as blood pressure monitoring
- Venepuncture and phlebotomy

Our team of 7 partners, 8 salaried GPs, 5 nurses, 4 HCAs, and a clinical pharmacist work together to provide a comprehensive range of services, including first contact physio and GP Support officers who deal with housing, benefits, social services queries, and social prescribing.
Person Specification
The ideal candidate will have:
- Significant post-qualification experience as a Registered Nurse
- Excellent communication skills, able to communicate with staff at all levels
- Experience in chronic disease management, immunisation, and venepuncture
- Ability to work in a fast-paced environment and prioritize tasks effectively
