About the Role

We are seeking a skilled and compassionate Community Nurse to join our Surrey Heath Community Nursing Team. As a Community Nurse with HCRG Care Group, you will play a vital role in delivering high-quality care to patients in their own homes.

Main Responsibilities

  • Conduct holistic assessments, develop care plans, and deliver evidence-based care to patients in their own homes.
  • Work collaboratively with the multidisciplinary team to ensure seamless care coordination and effective communication.
  • Implement and maintain high standards of care delivery, adhering to organizational policies and procedures.
  • Provide education and support to patients and their carers, promoting independence and self-management.
  • Participate in quality improvement initiatives, contributing to the development of best practices and service excellence.
