We are seeking an experienced Specialist Nurse to join our expanding nursing team for Looked After Children within Leeds Community Healthcare NHS Trust. The successful candidate will be responsible for managing and monitoring the delivery of healthcare services to looked after children and young people, aged 0-17.
Key Responsibilities- Assess, plan, deliver, and evaluate patient-centered programs of care to a specialist level, ensuring practice is grounded in evidence-based theoretical and practical knowledge.
- Work with multi-agency teams to improve health outcomes for looked after children and their families.
- Develop and maintain expertise in given areas, with opportunities for professional development and growth.
- Manage own workload and that of the wider team, within agreed objectives.
- At least 2 years post-registration experience in a relevant field.
- Knowledge and experience in looked after children and safeguarding.
- Excellent communication, organizational, and planning skills.
- Ability to work autonomously and as part of a team.
