Nurse Care Specialist

At Alderbrook Care Home, we are seeking a skilled Nurse Care Specialist to join our team. As a key member of our nursing staff, you will be responsible for delivering high-quality care to our residents, ensuring their physical and emotional needs are met.
Key Responsibilities:
- Craft personalized care plans in collaboration with residents, families, and healthcare professionals.
- Ensure accurate and up-to-date care documentation.
- Lead and guide the care team to deliver exceptional care.
- Facilitate seamless transitions and ensure continuity of care.
- Monitor residents' health and well-being, managing specific health needs.
- Administer medications safely and in accordance with MAR sheets and protocols.
- Build strong relationships with residents, families, and healthcare professionals.
- Promote independence and encourage social interactions and activities.
- Champion respect and dignity for every resident.
About You:
- You are a Registered General or Mental Health Nurse (RGN or RMN) with an active NMC Nursing PIN.
- You have a passion for care and a dedication to improving the lives of our residents.
- You possess excellent communication skills and a calm, patient demeanor.
